Graduate Certificate Application Process:
Applying to a Graduate Certificate program is a three-step process.
Apply
Fill out the Departmental Approval form online, then, print, sign
and submit to Graduate Certificates. (If you have difficulty opening
this link, please update your .pdf software using the Adobe Reader 8
icon in the right-hand column.)
Enroll
Use the
FACTS Online
Non-degree Enrollment Form
if you are new to USF or have not taken a course at USF within the last
12 months.
Submit
If you wish to take classes on campus, you must submit the Immunization
Health History Form form
prior to registration.

Strategies for Applying
to a
Graduate Certificate
Visit the Graduate Certificate
information page. Review the admission and course requirements for
the desired certificate. Print the certificate information page and keep handy for use when
checking the course schedule to see which graduate certificate classes
are offered for a given semester.
Call or e-mail the department contact to confirm
that the certificate fits your academic needs, to determine eligibility
for admission, or to find out if the department has any additional
requirements for admission.
Department
Approval Form Package
Submit the Department Approval Form with
letter of interest, resume, and other documents as stated under
"Admission requirements" and "Application process" on the Graduate Certificate
information page.
- Official transcripts, showing receipt of the requisite degree
should be sent, signed and sealed, directly to Graduate Certificates
in SVC1072.
- Students who received the requisite degree from USF are not required
to submit transcripts.
- Graduate students who submitted official transcripts showing the
requisite degree with the Graduate Admission application do not need
to re-submit transcripts.
- International students who do not hold the required degree from
a regionally accredited U.S. institution must submit translated
transcripts along with a letter from a qualified
foreign
transcript evaluation service certifying equivalency to the U.S.
degree.
- If applying to multiple Graduate Certificates, submit a separate
Department Approval Form, letter of interest and resume for each.
Notification of Admission:
Applicants will be notified in writing of acceptance or denial into a graduate certificate program. Admission into a graduate certificate does not imply future admission to a graduate degree program.

FACTS Online Enrollment Form
Are you new to USF, or returning after a 12-month lapse? In addition to submitting the Departmental Approval package
above, complete the FACTS Online Non-degree Enrollment
Form.

Submission of the Non-degree Enrollment form will allow USF to
establish a student account and U-ID number in OASIS, the
university's on-line registration system. Also use this form to re-activate an existing
account and U-ID if you haven't taken a course at USF in the last 12
months.
Immunization Health History Form
As a pre-requisite to registration, the State University System of
Florida requires all students born after 1956 to present documented
proof of immunity to Measles and Rubella. Submit the Immunization
Health History Form with proof of
immunization if you were born after 1956. Florida law also requires that
all admitted Florida university students be aware of memingococcal meningitis and hepatitis B,
two diseases that may be prevented by vaccination. Please review the Instructions at USF's Student Health Services web site for
information.
- Returning students need not re-submit proof of
immunization if prior proof is on file in the USF Student Health
Services’ Immunization Office. Contact 813-974-4056 if you are
unsure if your immunization record is on file.
- International students who will be entering the U.S.
while enrolled in either on-campus or on-line courses at USF must provide proof of health insurance to USF. If an international student is not on a student
visa, only proof of insurance coverage is required. If an international
student is on a student visa, specific coverages are required. For
